The Clinical Care Manager (CCM) provides care coordination and clinical management for MassHealth members with complex medical and behavioral health needs. The CCM is at the helm of organizing and coordinating resources and services in response to the enrollee’s healthcare needs across multiple settings and inclusive of both LTSS and Social Determinants of Health (SDH) needs.

The LPC Care Coordinator provides intensive care coordination and clinical care management for MassHealth members with complex medical and behavioral health needs who are enrolled in an Accountable Care Organization (ACO) or Managed Care Organization (MCO) plan. The LPN will partner with the Community Partner team and the clinical staff of enrollee’s ACO/MCO to promote integrated care and to ensure the quality and continuity of care.

The Team Leader provides leadership and oversight of all clinical and administrative functions of the Homeless Outreach Team.  The Team Leader coordinates outreach, assessment, service planning, referrals and community linkages for people with histories of homelessness and mental health  conditions. Position available on Cape Cod.

The Team Nurse promotes the full integration and functioning of the persons served that is needed to live a healthy life in the community. The Team Nurse coordinates access to and management of health care for the person receiving services in conjunction with Care Coordination entities and associated medical and behavioral health providers.

The Program Nurse functions in a direct care nursing capacity and consults with other clinicians regarding health promotion and maintenance, medication administration, and acute and chronic medical/psychiatric crisis management for persons served.
